KPI FORM – SERVICE QUALITY ASSURANCE
This form is used to assess the performance of a security officer based on key service quality indicators. It’s typically completed by a supervisor or operations manager (OM/OE) and acknowledged by the employee and management.
1. Basic Information

2. KPI Evaluation Criteria
Each category below must be graded using one of the following options:
Poor, Unsatisfactory, Satisfactory, Good, Excellent

Attendance | Punctuality, presence on duty, absence records |
Bearings & Discipline | Professional appearance, behavior, and attitude |
Code of Conduct | Compliance with company rules, regulations, and ethical standards |
Communication & Performance | Interaction with team, public, and management; overall job performance |
Knowledge | Understanding of site-specific procedures and security protocols |
Safety | Awareness and application of safety procedures and emergency protocols |
Training | Completion of mandatory training and ability to apply learned skills on the job |
3. Signatures Section
- Employee Signature: Confirms they’ve reviewed and acknowledged the KPI feedback.
- OM Signature: Signed by the Operations Manager or Executive conducting the evaluation.
- Management Signature: Final approval from upper management or HR.
Purpose of the Form
- Monitor officer performance regularly.
- Identify areas for improvement and training.
- Ensure service standards are met at deployment sites.
- Maintain a fair and documented performance review process.
